Henrietta Johnson Medical Center (HJMC), a Federally Qualified Health Center, provides a wide range of medical and dental services, at two sites located within the city of Wilmington. We offer caring, quality healthcare for children, adolescents, adults, and seniors. Every patient who visits our facility has his or her own dedicated physician. HJMC participates with most insurances, including Medicaid and Medicare. A sliding fee scale, based on income and family size, is available for uninsured patients.

Community health centers like ours have been recognized as a cost-effective and necessary alternative to emergency room-based primary healthcare services.

WHAT IS A MEDICAL HOME?

A "medical home" is a medical office or clinic where a team of health professionals work together to provide a new, expanded type of care to patients. Having a medical home feels like having a family doctor, but with a team of professionals, using modern knowledge and technology, to provide you the patient with the best possible care in the office.

HOW A MEDICAL HOME WORKS FOR YOU

In a medical home your care team:

  • Is available 24/7 if you need them. You can communicate with your care team by phone or email, and you can get an appointment quickly, even on the same day when needed.
  • You care team knows you and remembers your health history. They know enough about your personal or family situation to suggest treatment options that make sense for you.
  • Your care teams makes you understand your condition(s) and how to take care of yourself. They help you sort through your options and make decisions about your care.
  • Finally, your care team helps you coordinate your health care - even if they are not giving the care themselves. They will help you find your specialists, get appointments, and make sure your specialists have all the information they need. Your medical home team will also be sure you know what the specialists say and what it means for you.

June 2010 Healthy Insights

A breakfast was held in March in the Wilmington Trust Company boardroom to educate targeted community leaders about the healthy contributions HJMC makes to its community. U.S. Representative Michael Castle attended this important event.

HJMC is pleased to announce the addition of two new medical providers to its staff.

  • Flauvius Akerele, M.D. is a board-certified OB/GYN with more than 30 years of experience in all areas of women’s health.
  • Sheri L. Brown, MSN, RN, FNP-C is a Licensed Family Nurse Practitioner who works collaboratively with the whole HJMC medical team to diagnose and treat a variety of diseases and injuries in general practice.

Delsey Morales, HJMC patient navigator has been named “Youth of the Year” for the State of Delaware. Congratulations Delsey—we’re all proud to have you be a part of the HJMC “family”.

 

Henrietta Johnson Medical Center Opens Expanded Dental and Women's Health Areas at it's Southbridge location. . . . .

Henrietta Johnson Medical Center unveiled its expanded patient services areas for dental and women's health with an Open House on Monday, October 22, 2012 attended by Governor Jack Markell, Senator Tom Carper and city, county and other officials and funders. The expansion also includes a new community meeting room/classroom and renovated nursing space for family health services.
